An incident investigation is an opportunity to understand how work happens, what people were dealing with and where the organisation can make meaningful improvements. Yet investigations can stop at familiar explanations: someone made an error, missed a step or didn’t follow a procedure. Those findings leave important questions unanswered. This practical one-day masterclass helps you look deeper. Drawing on operational investigation experience and the principles of Human and Organisational Performance and Safety Differently, we explore how to understand decisions and actions in the context in which they occurred. We work through the investigation process, from planning and gathering evidence to having learning conversations, making sense of different accounts and developing useful findings. You’ll explore how hindsight and assumptions can shape an investigation, how to follow threads that deserve further attention, and how to involve the people who know the work in checking your understanding. Through practical examples, discussion and activities, you’ll build your ability to: • Plan an investigation around clear questions and opportunities for learning. • Gather evidence that reveals the pressures, constraints and resources shaping the work. • Ask questions that encourage people to share their experience. • Analyse information without rushing to a simple explanation. • Develop recommendations that address the conditions identified. • Communicate findings that help leaders make better-informed decisions. The masterclass is designed for investigators, safety professionals, operational leaders, supervisors and anyone responsible for leading, supporting or reviewing incident investigations.
